FEDERAL – The library will only receive 1040/1040-SR Forms and Instructions.

The 1040/1040 SR Forms booklet includes:

  • Form 1040 and Form 1040-SR (For Seniors)
  • Schedule 1 (Additional Income and Adjustments)
  • Schedule 2 (Additional Taxes)
  • Schedule 3 (Additional Credits and Payments)

MINNESOTA - The library does not receive tax forms from the state.

To access Minnesota tax forms and instructions:

    • Download and print forms: https://www.revenue.state.mn.us/find-form
    • Call 651-296-3781 or 1-800-652-9094 to have forms mailed to you
    • Pick up forms at: Revenue Dept, 600 N. Robert St., St. Paul, MN, 55101 (M-F, 8-4:30)
